Harlington Hospice Care comprises of a team of trained Health Care Assistants that provide appropriately timed quality care and support to patients who have long term conditions but are medically stable.

The main aim of the service is to facilitate earlier discharge from hospital, providing an appropriate tailor-made care package for up to ten days until a care package has been sourced from another agency.  

Our Service

Our team of staff provide personal care up to four times daily with the support of one or two members of staff. Our service is available 365 days a year on a daily basis. (including weekends and bank holidays).

We also provide long term service provision for those patients who are seriously ill and wish to remain in the comfort and familiarity of their own homes.

We provide:

  • Medication administration and prompting
  • Food preparation and assistance with meals, feeding, PEG feeds
  • Personal care
  • Washing patient’s soiled linen, towels and clothes
  • Safe moving and handling of patients
  • Liaison with other services for equipment or additional support

Who can refer to Harlington Hospice Care?

  • Referrals to the service are made via Social Workers based at The Hillingdon Hospitals NHS Foundation Trust and London Borough of Hillingdon Social Work Teams. for more details on how to make a referral please click here
